Step 1: Register for Your Project
If you have not yet registered, please select the registry that applies to you / your family to begin the enrollment process. It only takes a few minutes.
Once you complete the registration form, you will receive a welcome email containing a password for the consent page and a secure, personalized upload link to upload your consent.
